ABSTRACT
The prevalence of dental caries among children under five in Indonesia reaches 90%, with rampant caries being one of the most concerning forms, particularly resulting from prolonged bottle-feeding habits. Maternal knowledge, as the primary caregiver, plays a critical role in shaping early childhood oral health behaviors. This study aimed to analyze the relationship between mothers' knowledge regarding bottle-feeding habits and the incidence of rampant caries among toddlers at TK RA/KB Da'arul Qur'an Kediri. This study employed an observational analytic design with a cross-sectional approach. Subjects consisted of 31 mother-toddler pairs aged 3–5 years, selected using a total sampling technique. Maternal knowledge was collected through a structured 20-item questionnaire, while rampant caries was assessed via clinical examination using the def-t index and caries type classification (Type I–IV). Statistical analysis was performed using Spearman's correlation test with a significance threshold of p<0.01. The majority of mothers demonstrated moderate knowledge (80.6%), while 87.1% of children exhibited Type IV (severe) rampant caries with a mean def-t score of 8.7, classified as very high according to WHO criteria. Spearman's test yielded p=0.43, indicating no statistically significant relationship between maternal knowledge and rampant caries incidence. These findings suggest that knowledge alone is insufficient without accompanying attitudinal and behavioral changes in daily caregiving practices. No significant relationship was found between maternal knowledge of bottle-feeding habits and rampant caries incidence among toddlers. A holistic approach is required and integration of dental health education into preschool curricula.
